You will need to identify, with the help of your preceptor and/or field site, the appropriate individual at the field site that can speak to the affiliation agreement. This is usually the office manager or someone in risk management, the legal department, HR, an attorney, etc.

You will be asked to provide the contact information of this individual in your practicum application. The Field Education Coordinator will work with the correct individual to finalize the affiliation agreement, if possible. Note most agreements can work out but back up sites are recommended if a legal review is needed.

 

What is an Affiliation Agreement?

An Affiliation Agreement is a legal contract between Walden University and a field experience site or its legal entity.  An Affiliation Agreement addresses the responsibilities of each party regarding field experience placements.  Walden University requires a current Affiliation Agreement to be in place before students may start their field experience.

An Affiliation Agreement is not the same as the preceptor signature form. Do not confuse these two.

 

Identify the Field Site Administrator

  • Work with your preceptor to determine who this is
  • They can speak to Walden in regards to a legally binding Affiliation Agreement
  • They may be the CEO, Dean, Contracts Administrator, Site Director, Human Resources Manager, legal council, etc.
