You will need to identify, with the help of your preceptor and/or field site, the appropriate individual at the field site that can speak to the affiliation agreement. This is usually the office manager or someone in risk management, the legal department, HR, an attorney, etc. You will be asked to provide the contact information of this individual in your practicum application. The Field Education Coordinator will determine if an agreement is already on file. If not, the field coordinator and contract administrators (and legal team if needed), will work with the correct individual(s) to finalize the affiliation agreement, if possible. Note most agreements can work out but back up sites are recommended if a legal review is needed.
