How can I request a FERPA form?

Electronic Format

1. Login to your myWalden portal.
2. Go to your myFinAid Portal on the Finances tab.
3. Under Quick Links, click “View Your myFinAid Account".
4. Click on the Menu and select Forms.
5. Select "click here" to access the myFinAid Document Portal.

Note: If this is your first time accessing you will need to verify your information and login

6. You will now see the myFinAid Document Portal Welcome Page. Click Request.

6. Select the green plus sign for the FERPA form.
7. Click download, print the form, complete and sign, upload.
8. Click Finish.

Paper Format

Please submit your request using the FERPA form.
