The most current status for your affiliation agreement will always be listed in your application’s checklist under the “Affiliation Agreement” section—please see below for a breakdown of the various statuses:
Site information submitted by student: This means that the coordinator has received your information and is checking to see if we have a valid agreement on file. If we don’t the field site administrator will be notified. If we do the coordinator will upload the agreement to your application.
Field site administrator notified : This means the field site administrator that you indicated in your application was emailed asking which agreement option they would like (i.e., already exists, covered site, use Walden University Affiliation Agreement unchanged, etc.); this email has login credentials for them to sign in to Meditrek and complete information on their field site and agreement.
You should also verify that the email address you entered for your field site contact is accurate and in the proper format (with no extra spaces before or after the email address).
Draft saved by Field Site Administrator: This means the field site administrator has begun selecting an agreement option and/or has begun entering address information, but has saved as draft instead of submitting their selection. They should log in and complete their selection.
Affiliation Agreement submitted to Walden for review: This means the field site administrator has logged in and submitted information; either they have been sent a copy of our standard agreement to sign, they have selected that an agreement is already on file or is a covered field site or they have been directed to our legal team via an auto-email (this will either ask them for their field site’s standard template or if they want to make changes to the provided standard Walden agreement).
Affiliation Agreement uploaded: This means the agreement has been uploaded to your application, BUT it has not yet been approved or rejected.
Affiliation Agreement completed and approved: This means an agreement is on file with your chosen field site and is valid and approved.
